Web form opt-in involves a form on your website where users enter their phone number and explicitly agree to receive text messages from you.

Required Documentation:

You must provide the direct URL to the web form. Alternatively, you can submit a screenshot of the web form.

Key Requirements for Web Forms:

  • Phone Number Field: The form must have a field specifically for the user's mobile phone number.

  • Explicit Consent: There must be a clear and unambiguous opt-in mechanism, such as a checkbox. This checkbox must be selectable by the user and cannot be pre-selected. Pre-checked boxes are not considered valid consent.

  • SMS Opt-in Only: The form should only collect consent for SMS messages. Opt-ins for email addresses or other communication methods are not acceptable for toll-free number verification.

  • Clear Language: The opt-in language must clearly state that the user is agreeing to receive text messages. Include information about:

    • Message frequency (e.g., "You will receive up to 4 messages per month").

    • Message and data rates (if applicable).

    • How to opt out (e.g., "Reply STOP to opt out").

    • Link to your Privacy Policy and Terms of Service.

Example Web Form Language:

Enter your mobile phone number to receive updates and special offers from John's Coffee Shop! Phone Number: [Phone Number Field] I agree to receive text messages from John's Coffee Shop. Message and data rates may apply. Reply STOP to opt out. [Link to Privacy Policy] [Link to Terms of Service] [Submit Button]

Acceptable Submissions:

  • Direct URL to the web form.

  • Screenshot of the web form showing the phone number field, opt-in checkbox (unselected), and required disclosure language.

Unacceptable Submissions:

  • URLs that do not lead directly to the opt-in form.

  • URLs that are behind a login screen or require a password.

  • Screenshots of pages other than the opt-in form.

  • Forms that pre-select the opt-in checkbox.

  • Forms that collect email opt-ins only.

  • Forms where the opt-in language is unclear or insufficient.

Best Practices:

  • Make the opt-in process clear and easy to understand.

  • Use concise and straightforward language.

  • Ensure the form is mobile-friendly.

  • Keep the form focused solely on SMS opt-in.

NOTE: If you choose WEB_FORM, you must provide the link to it in the OptInImageURLs parameter
