What is Paper Form Opt-in?

Paper form opt-in involves customers filling out a physical form in person to give their consent to receive text messages. This is common for in-store sign-ups or events.

Required Documentation:

You must provide a scanned copy or clear photograph of the physical form.

Key Requirements for Paper Forms:

  • Clear Consent Language: The form must clearly and explicitly state that the user is consenting to receive text messages. Avoid vague language.

  • Phone Number Field: The form must have a dedicated field for the user's mobile phone number.

  • Opt-in Checkbox/Signature: There should be a clear way for the user to indicate their consent, such as a checkbox or signature line specifically for SMS opt-in. If a signature is used, the language near the signature must explicitly mention text message consent.

  • Required Disclosures: The form must include the same disclosures as web forms (message frequency, message/data rates, opt-out instructions, privacy policy link, terms of service link).

  • Date: The form should include a date field to record when the consent was given.

Acceptable Submissions:

  • Clear scan or photograph of the completed paper form, showing all required information.

Unacceptable Submissions:

  • Forms that do not explicitly mention text message consent.

  • Forms that are incomplete or illegible.

  • Forms without a phone number field.

  • Forms where the opt-in is implied or unclear.

  • Forms that only collect email opt-ins.

Best Practices:

  • Use clear and easy-to-read font and layout.

  • Ensure there is enough space for users to write their information legibly.

  • Store completed forms securely.

  • Train staff on how to explain the opt-in process to customers.
