Key Requirements for Paper Forms:

Phone Number Field: The form must have a dedicated field for the user's mobile phone number.
Opt-in Checkbox/Signature: There should be a clear way for the user to indicate their consent, such as a checkbox or signature line specifically for SMS opt-in. If a signature is used, the language near the signature must explicitly mention text message consent.
Required Disclosures: The form must include the same disclosures as web forms (message frequency, message/data rates, opt-out instructions, privacy policy link, terms of service link).
Date: The form should include a date field to record when the consent was given.
